(Faculty)
By accepting possession of the computer, peripherals and software (equipment), I agree to the following terms and conditions:
Throughout the agreement, I shall use the equipment in accordance with the South Carolina State University policies on the appropriate use of computer resources. I DO NOT OWN THE EQUIPMENT; I have a "license" to use it only. I AM REQUIRED TO RETURN THE EQIPMENT TO THE UNIVERSITY AS DESCRIBED BELOW. I shall not permit any other person to possess or use this equipment. Commercial and illegal use of this equipment is prohibited.

If I do, I may be charged repair or damage fees not to exceed $1000.00.
-
I understand that I may be required to check-in my laptop periodically throughout the semester, and that I will be notified of the check-in details via University email.
-
It is my responsibility to give written notice to the Project ASSET Office of any change in my status as a South Carolina State employee.

I understand that I will be charged a fee for the replacement of any stolen equipment up to $1,000 per incident that is not documented by a police report. I understand that having an official police report does not necessarily release nor exempt me from charges for the replacement of the stolen equipment.
-
I understand that all repairs must be made through UCITS PC Support Services.
-
I understand that if there is a hardware or software problem with my laptop that the technician is not able to resolve immediately, I will turn in my laptop and, if available, I will be issued a loaner unit until problems with my laptop are resolved.
-
I agree not to make any alterations, hardware additions, or enhancements to the laptop. Such alterations include, but are not limited to, additional memory or removal of any hardware/software included with the computer. Removal or altering of any University issued software is PROHIBITED.
-
I understand that UCITS PC Support Services will only support the University Standard hardware and software. If there is a software issue that requires that the laptop be re-imaged, PC Support Services will only restore academic data. I am responsible for restoring any of my own software or non-academic data.
-
I understand that I am required to use the laptop carrying case that was issued to me with my laptop and that I will be held responsible for any damage that occurs to the laptop as a result of my failure to use this case.
-
I understand that this laptop is being issued as part of my participation in Project ASSET; therefore, I will be required to participate in project evaluation activities.
