  • If paying with check or money order:
• Complete the CBT Authorization Voucher Request Form (PDF) and mail it to ETS. Allow 3 weeks for delivery of the voucher. When you receive the voucher, call Prometric at 1-800-853-6773 or 1-443-751-4859 within 90 days to schedule your test. You must bring the voucher to the test center.
• You may only take the Praxis I® computer-based test once per calendar month and no more than 6 times within a 12-month period. See the Praxis™ Information Bulletin (PDF) for complete policies.
