| Complete the CBT Authorization Voucher Request Form (PDF) and mail it to ETS. Allow 3 weeks for delivery of the voucher. When you receive the voucher, call Prometric at 1-800-853-6773 or 1-443-751-4859 within 90 days to schedule your test. You must bring the voucher to the test center. |
