Audio-Visual Equipment Request Form
Requests for audio visual equipment must be submitted at least five (5) working days in advance of the first day of the course

Copying Request for Handouts/Materials
Materials to be copied must be received no later than five (5) working days in advance of the first day of the course. Copyrighted materials will not be reproduced unless the Facilitator provides appropriate documentation indicating that the material can be copied.

Course Evaluation Results
Facilitators will receive within three to five days after the completion of their recent course(s) a link to review a summary of the course evaluations.
